3-2 Milestone One: Draft of Analysis of Regulations. Before you begin this milestone, read the related documents located in the Assignment Guidelines and Rubrics section of the course. Based on the scenario given in the Final Project Document, and using the SUNY Downstate Medical Center Safety Plan for reference, submit a draft of your analysis of the regulation(s) that address the safety issue you chose in the given scenario. Your analysis should include the identification of healthcare regulations or accreditation standards related to the safety issue and the analysis of their impact on the given organization. In this final project you, conduct an analysis of regulations that impact a given scenario, a gap analysis of the given department, followed by recommendations for compliance. You will use the SUNY Downstate Medical Center Patient Safety Plan along with the given scenario to determine if they have the proper accreditation or regulatory compliance. You will also recommend corrective action strategies for remaining compliant with appropriate regulations. Scenario: You walk into the intensive care unit. There are no patients or family members sitting in the chairs; the patients in bed are on ventilators, attached to all kinds of tubes. The nurses are dressed in various colors of scrub suits. You notice two sedated patients on ventilators who have at least one side rail down on the bed. One of these beds is also elevated. Tubes are visibly seen attached to machines for monitoring and maintaining physiologic functions such as blood pressure, oxygen levels, and nutritional support. No labels are noted on the tubing. Patient armbands are taped to the side rails instead of to the patient. In a rush to take care of multiple patient needs, nurses go from one room to another with gloves on. The noise level consists of alarms going off intermittently, the whir and hum of machines, and the spoken technical jargon of staff. The physicians and nurses are talking in a language that is specialized in their area. "Mr. Smith has multifocal PVCs (premature ventricular contractions) and PCWPs (pulmonary capillary wedge pressures) are high. Mr. Smith is hypotensive, tachycardiac, and is receiving titrating IV medications to increase urine output and blood pressure." There are observable clusters of care in the intensive care unit. As the physicians round with the nurses and ancillary staff, the language of each specialty is heard. There is an urgency and quickness of pace. Emergent care interrupts the rounds, and the attending physician, residents, and nurses rush to the bedside of a patient who has "coded." Gap Analysis: In this section, you will conduct a gap analysis and explain the key elements of any compliance plans that are in place (or should be in place). Be sure to: A. Explain the process that is in place or should be in place for approaching regulatory or accrediting compliance. For example, what actions has the department taken to ensure compliance? What are the procedures or strategies employed or that should be employed by the department to maintain compliance? B. Determine current or potential gaps in the regulatory or accrediting compliance of the healthcare department or system. In other words, is the department compliant with appropriate regulations or accreditation? What gaps are present regarding compliance? III. Recommendations: In this section, imagine you are preparing recommendations for your supervisor regarding the regulatory or accreditation compliance of the department. Specifically: A. Recommend corrective action strategies that address your described gaps and meet the compliance requirements of the regulating bodies or accrediting agencies that would apply to the department or system. B. Defend your strategies by explaining the implications for the department or system if the previously identified gaps are not addressed. In other words, what could happen to the department if it did not follow your recommendations? C. Explain how you would establish an accreditation or compliance program for the department or system. Be sure to relate implementing your program with previously identified gaps. In other words, explain how you would implement your recommendations regarding accreditation or compliance program. D. Explain how you would collect meaningful data to track compliance with your corrective action plan. Consider different ways to go about tracking this compliance.
