My Chosen Planning Stage: Patient with Mental Health Issue in Scotland, UK – Depression.

My placement area: Assessment and Rehabilitation Centre, day hospital for people over 60. We’re a multidisciplinary team that offer physiotherapy, occupational therapy, nursing staff and doctors. The aims to maximize patients’ potential and empower them to manage long term situations in their own home. Falls assessment, to try and prevent admission to hospital. Applications for day clinic can be made through GP, district nurse or social work department. Some of our patients have cognitive difficulties or visual or hearing impairment. My chosen activity task: to assist a patient into the bathroom - toileting/personal care.
Patient’s Problem List: Mental Health Issue with increasing falls, and possible depression due to bereavement. Patient has a walking stick, able to walk with the assistant of one staff. Aim of activity: ensure the patient hygiene needs are being met. Plan activity date: Feb. 4, 2019. Carried out within my placement day hospital.
As a student, I must ensure I work within my limits and capabilities. I must ensure my learning, skills and experience are put into practice. This will ensure I complete my activity competently. My responsibilities are to ensure the patient is kept safe, comfortable, preserving their dignity, also promoting personal hygiene.
If any unforeseen circumstance were to happen and I become unable to carry out my first planned task, I have a contingency plan in place that will be carried out on the same day. My contingency plan is to assist the same patient with a pre-bladder scan. I will submit my development stage and evaluation stage on the agreed dates.
