Case Study In institutions, caregiving is generally focused on efficiency (getting the job done) rather than on identifying and supporting the cognitively impaired (CI) person's retained abilities with activities of daily living (ADLs). This practice accelerates the loss of abilities, induces excess disability, and poses a threat to autonomy and self-concept.
To break this cycle, an urgent need exists to develop strategies that maintain or improve ADL independence in CI adults for as long as possible while meeting the institution's demands for efficiency. Previous studies have identified behavioral strategies used by caregivers during ADLs.
Strategies to promote independence in dressing were used in the current study because dressing involves a predictable series of tasks and successful use of the strategies with dressing might encourage caregivers to generalize the strategies to other ADLs. A behavioral intervention, Strategies to Promote Independence in Dressing (SPID), was tested. In pilot studies, SPID was found to improve the dressing independence of CI nursing home residents. In SPID, the level of assistance provided by the caregiver is matched with the cognitive abilities and deficits of the resident. Two hypotheses were tested: There will be a significant improvement in the dressing independence of CI nursing home residents following implementation of SPID.
There will be no difference in the time required by nursing assistants to complete dressing activities with CI residents before and after implementing SPID.
